Specimen Requirements
BIOPSIES (GYN & NON-GYN)
Place each specimen in a pre-filled formalin container.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
CULTURES
Testing not performed at IPMG, specimens forwarded as a courtesy.
Submit specimen in a sterile container or a sterile culturette swab. Call lab if unsure of how to submit.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Place container with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
Call for courier pick-up; specimens for culture must be picked up the same day.
Specimens such as fluids submitted in a sterile container with no preservative should be refrigerated until pick up. Specimens submitted using a culturette swab should remain at room temperature.
FNA /NEEDLE BIOPSY
Place each specimen in a pre-filled formalin container.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
FNA/ FLUID
Place each specimen in a pre-filled CytoLyt vial.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
PROSTATE BIOPSIES
Place each specimen in a pre-filled formalin container.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Prostate biopsy kits available from Interscope (contains twelve 20 ml. formalin containers in an organized box).
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
URINE CYTOLOGY
Place freshly voided urine specimen in a urine container.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
Place in refrigerator until specimen is picked-up.
UROVYSION
Submit freshly voided urine specimen in a sterile container.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Call for courier pick-up; specimens for UroVysion must be picked-up the same day.
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
Place in refrigerator until specimen is picked up.
CALCULUS—(STONES)
No Fixative.
Place in a dry container.
Label each specimen separately with the full last and first name of the patient and the source/site of the specimen.
Place containers with a completed tissue/non-gynecologic (blue) requisition form in an appropriate biohazard transport bag and seal.
